$35 Sports Physicals for Student Athletes

No. Sports Physicals are available as a standalone service. The athlete does not need to be an established patient or a member of the practice.

This service is available to student athletes through high school who need an MHSAA sports participation physical.

The cost is $35. Payment is due at check-in on the day of the scheduled Sports Physical. This is a self-pay service and will not be billed to insurance.

Please call the office at 269-786-6010 to schedule. Sports Physicals are available during regular scheduling availability and are not offered as a walk-in service.

The athlete and a parent or legal guardian must attend. Both will be asked to complete and sign the practice’s Sports Physical consent form before the examination begins.

Please bring the athlete’s required MHSAA form, student ID card, and $35 payment. Families should obtain the current form from the athlete’s school, athletic program, or the official MHSAA website before the scheduled Sports Physical.

The service includes a sports participation physical performed by Dr. Borenitsch and completion of the athlete’s MHSAA form.

No. Dr. Borenitsch will determine participation clearance based on the athlete’s medical history and physical examination. Additional evaluation or documentation may be required before an athlete can be cleared.
